Globally, complications of prematurity remain a leading cause of neonatal mortality, with respiratory distress syndrome (RDS) accounting for a significant proportion of preventable deaths. In low- and middle-income countries (LMICs) like Bangladesh, access to effective respiratory support is constrained by high costs, dependence on electricity and compressed air, limited technical capacity, and equipment maintenance challenges. Although Bangladesh has achieved substantial reductions in under-five mortality, neonatal mortality—particularly due to preterm birth and low birth weight—has declined more slowly. To address this gap, bubble Continuous Positive Airway Pressure (bCPAP) has been integrated into the national newborn health strategy. The Vayu-bCPAP system, an ultra-low-cost, FDA Emergency Use Authorized device that functions without electricity or compressed air, offers a context-appropriate solution. Combined with an innovative low-flow oxygen blender, the system enables safe oxygen delivery and reduces risks such as retinopathy of prematurity. Evidence from multiple countries demonstrates improved survival outcomes, highlighting Vayu-bCPAP’s potential to strengthen neonatal care and reduce preventable newborn deaths in resource-limited settings.
